The family registry system is a record of births, deaths, marriages and other information.
In accordance of the family registration law, foreign national residents must notify city hall or town hall of any  births, deaths, and marriages.

2-1 Birth registration
(1) Apply within 14 days after the birth (within 3months after the birth, if a child of Japanese spouse was born outside of Japan).
(2) Applicant shall be either father or mother.
(3) Applicant shall apply to birthplace, present address of applicant, or permanent address of spouse of Japanese national.
(4) Required items to be submitted are:
A. Birth Registration Form with Birth certificate
B. Passport of parents, if they are foreign nationals
C. Registration card
D. Marriage Certificate with Japanese translation attached is required, if both parents are foreign nationals.
E. Maternal and child health handbook
F. National health insurance certificate (those enrolling)
G. Seal of applicant (if applicant is Japanese national)

2-2 Notification of death
 Notification of death shall be made as follows
(1) Apply within 7 days after the person has passed away or was confirmed to be dead.
(2) Applicant shall be the family member, those who live together, land owner, caretaker, conservator, or legal guardian such as guardian ad litem.
(3) Office to be applied is city, ward, town or village where dead person lived or applicant lives
(4) Required items to be submitted are:
A. Death Registration Form with Death certificate
B. Registration cards (of dead person and applicant)
C. Passport (of dead person and applicant)

2-3 Marriage registration
  The following items are required for marriage registration
(1jMarriage registration form (signatures of both parties,  signatures and/or seals of two adult witnesses are required)
(2) Certificate of Legal capacity to contract marriage with Japanese translation attached, which is issued by the local government of his/her country or embassy .
(3) Passport with Japanese translation attached
(4) Birth certificate with Japanese translation attached
(5) Foreign resident registration card
(6) Seal of Japanese spouse
(7) Family registry of Japanese spouse (if he or she registers in his or her legal domicile, it is not required)

2-4 Divorce registration
 The following items are required for divorce registration.
(1) Divorce registration form (signatures of both parties,  signatures and/or seals of two adult witnesses are required).
(2) Family registry of Japanese spouse
(3) Passport
(4) Certificate of residence of Japanese spouse (certificate of registered matters of foreign spouse, if Japanese spouse is outside of Japan )

2-5 Naturalization
  To acquire Japanese nationality, you must obtain permission from the Minister of Justice. Then you must notify the city hall.
The folllowing documents are required to naturalization registration.
i1j Naturalization registration form (if you have a Japanese spouse, his/her signature or seal is required.)
(2) Your identification card
(3) Family registry of Japanese spouse (if he or she registers in his or her legal domicile, it is not required).
